BELOW ARE SOME OF THE DOCUMENTS NEEDED FOR ASSISTANCE
:
·        APPLICANT PICTURE ID
·        PROOF OF INCOME, IF NOT WORKING, BRING STATEMENT FROM 
            UNEMPLOYMENT OFFICE (BENEFITS HISTORY FORM)
·        EVERYONE 18 yrs old OR OLDER AND NOT RECEIVING INCOME MUST PROVIDE
BENEFIT HISTORY PRINTOUT FROM UNEMPLOYMENT OFFICE
·        SOCIAL SECURITY CARDS FOR ALL IN HOUSEHOLD MEMBERS
·        PLEASE BRING RECEIPTS OF MAJOR BILLS PAID IN LAST 30 DAYS
·        PLEASE BRING EVICTION LETTER/NOTICE FROM LANDLORD (EVICTION NOTICE CAN BE a NOTARIZED LETTER OR a COURT ORDERED EVICTION NOTICE)
·        FOR MORTGAGE, PLEASE BRING LETTER OF DEFAULT or LATE NOTICE
